For each visitor to my web site, my web server automatically recognizes only the visitor’s originating domain name, but not the e-mail address (where possible).

We collect aggregate information on what pages visitors access or visit, information volunteered by the visitor, such as survey information and/or site registrations, name and address, telephone number, payment information (e.g., credit card number and billing address) and computer model, operating system type and version, web browser type and version.

Customers have the option of opting in or out of my mailing list during appointment scheduling. If you do not want to receive e-mail from me in the future, please let me know by opting out of the list when you’re scheduling an appointment or by sending me an e-mail at the above address.

If you supply me with your postal address on-line you will only receive the information for which you provided us your address (e.g., a copy of your invoice or other requested information).

Persons who supply me with their telephone numbers on-line will only receive telephone contact from me with information regarding orders they have placed for my services.

With respect to ad servers: I do not partner with or have special relationships with any ad server companies. From time to time, I may use customer information for new, unanticipated uses not previously disclosed in my privacy notice. If my information practices change at some time in the future I will post the policy changes to my web site to notify you of these changes and provide you with the ability to opt out of these new uses. If you are concerned about how your information is used, you should check back at my web site periodically.

Upon request I will provide site visitors with a description of information that I maintain about them. visitors can access this information by e-mailing me at the above address.

Upon request I offer visitors the ability to have inaccuracies corrected in contact information and transaction information. Visitors can have this information corrected by sending me an e-mail at the above address.

With respect to security: I believe I have appropriate security measures in place in my physical facilities to protect against the loss, misuse or alteration of information that I have collected from you at my site.
